GENERAL INFORMATION NOTICE

ATAGÖZ Private Healthcare Services Inc. (“ATAGÖZ MEDICAL CENTER”), which operates in the field of healthcare services, also demonstrates its sensitivity to the security of personal data.

With this awareness, we place great importance on processing, storing, and deleting both special and general personal data of our visitors on all platforms in compliance with the Law No. 6698 on the Protection of Personal Data (KVKK), its secondary legislation (regulations, communiqués, circulars),and the binding decisions of the Personal Data Protection Board.

In line with this responsibility, as the “Data Controller” defined under the Law, we process your personal data as explained below and within the limits prescribed by the legislation.

INFORMATION ABOUT THE DATA CONTROLLER

Pursuant to the Law No. 6698 on the Protection of Personal Data, ATAGÖZ Private Healthcare Services Inc., registered with the Izmir Chamber of Commerce under registry number 114974, located at Çınarlı Mahallesi Fatih Caddesi No: 32, Konak – İzmir, is the Data Controller.

PURPOSES OF PROCESSING PERSONAL DATA

ATAGÖZ MEDICAL CENTER requires the processing of personal data both for fulfilling its legal obligations directly and for situations indirectly arising from these obligations and the services provided.

Accordingly, your personal data is processed by ATAGÖZ MEDICAL CENTER for purposes such as:

• Patient registration
• Obtaining a guarantee that no additional fees will be requested from SGK
• Billing and invoicing
• Patient identification
• Physical facility security
• Pre-examination preparations
• Post-examination procedures
• Performing surgeries
• Financial data processing
• Laboratory service notifications
• Procurement

These purposes are always applied in a relevant, limited, and proportionate manner.

TRANSFER OF PERSONAL DATA

Your personal data may be transferred to official institutions and organizations upon request, within the scope of the purposes listed above, and in accordance with Articles 8 and 9 of the Law.

METHODS OF COLLECTION AND LEGAL BASIS

Your personal data may be collected verbally, in writing, or electronically through automatic or non-automatic methods by authorized data processors of ATAGÖZ MEDICAL CENTER via e-mail, phone, website, various agreements, or paper forms and records.

In this context, personal data such as:

• Turkish ID number (TCKN)
• Parent information
• Contact details
• Place and date of birth
• Assigned doctor
• Prescriptions, medical rest or school reports
• Medical history
• Image recordings
• Palm data
• Surgical information

are processed based on Articles 5-2(a, c, e, f) and 6-3 of the KVKK.

RIGHTS OF DATA SUBJECTS

You may apply to ATAGÖZ MEDICAL CENTER at any time to:

• Learn whether your personal data has been processed, its purpose, and whether it has been used in accordance with that purpose, and request information if processed
• Learn the third parties within or outside Turkey to whom your data has been shared
• Request correction of incomplete or inaccurate data
• Request deletion or destruction of your data under the conditions of Article 7 of the Law
• Request notification of your correction, deletion, or destruction requests to third parties to whom your data has been transferred
• Object to any negative outcome caused by automatic systems or request compensation if your data has been recorded or used unlawfully and you have suffered damage

If there is an additional cost for your application, the Communiqué on Procedures and Principles for Application to the Data Controller issued by the Personal Data Protection Board determines the fees. The first 10 pages of written responses are free, with a fee of 1 TL per page thereafter. If the response is provided on a CD, flash drive, or similar medium, the cost of the medium will be charged.

Your requests will be processed as soon as possible and no later than 30 days depending on the nature of the request.

Applications can be submitted in Turkish in writing to ATAGÖZ MEDICAL CENTER via:

1. Completing the form at www.atagoz.com.tr, and submitting a signed copy in person or via a notary to Çınarlı Mahallesi Fatih Caddesi No: 32, Konak – İzmir
2. Completing the form at www.atagoz.com.tr, signing it with a secure electronic signature under Law No. 5070 on Electronic Signatures, and sending it via registered e-mail to atagoz@hs01.kep.tr

Applications must clearly state the request and be related to the applicant. If acting on behalf of someone else, a notarized power of attorney is required. Required details include full name, signature, Turkish ID number, residential or workplace address, e-mail, phone, fax, and request specifics, in accordance with the Communiqué. Applications lacking these elements will be rejected.

ATAGÖZ MEDICAL CENTER reserves the right to make changes to this notice due to obligations arising from the Law, secondary regulations, and Board decisions. Changes become effective immediately upon notification.
